Learning from incidents

Learning from incidents (LFI) is just one of many activities in managing safety in an organisation. Incident investigation is the best known part of the LFI process. Our services go beyond that. We help organisations optimise the LFI process and develop effective safety improvement actions.

Human factors

Human error is often cited as the ‘source’ of many accidents. While we acknowledge that human error is often the cause of incidents, we emphasise that human errors do not occur in isolation. They are influenced by external factors (organisational or environmental). Rather than focusing on blaming the individual who made the error, our approach focuses on the logical analysis of these ‘error-causing’ systemic influences.
